I had some surgery on my head a few years ago and, as a 32 year old man in NYC, would rather not have to live with the scars on my head. What possible solutions are out there to conceal the scars?

I have seen many patients successfully have hair transplanted into scar tissue and have wonderful results concealing the scar. While we can transplant hairs into scar tissue, the blood supply is different and therefore we run a higher risk of the hairs not regrowing.  It is a small chance that most patients are happy to take in order to cover an unsightly scar.
